Patients are required to fill out a pre-admission form and return it to the Day Surgery at least one week prior to admission. Our receptionist will then contact you and inform you about the admission process and payments required.

Dentists are required to arrange pre-admission tests for patients with medical conditions and all patients over 55 years of age.

Dentists may complete this form and send it with their patient to their GP to arrange pre-admission tests. Alternatively, tests can be requested by the Dentist using the appropriate pathology form.
